FOR HEALTH & CARE PROFESSIONALS
Referral & Clinical Support
If you’re a physician, nurse, social worker, care coordinator, or other healthcare professional working with seniors, you can refer patients to us directly. We make the process simple, prompt, and professionally coordinated.
WHY REFER TO SMDS
Oral Health Is Part of Overall Health
Poor oral health in older adults is associated with increased risk of pneumonia, cardiovascular complications, nutritional deficiencies, and reduced quality of life. For many of your senior patients, accessing a dental clinic is genuinely difficult or simply not happening.
We offer a simple referral pathway so that your patients’ dental health needs are addressed by a professional at their home or care facility, without the barriers of transportation or unfamiliar environments.
WHO CAN REFER
We Welcome Referrals From Any Healthcare Professional

👩⚕️
Nurses & Nurse Practitioners

🏥

Home and community care coordinators who support seniors with multiple health and social needs.
🤝

🏘️

👨👩👧
Families & POAs
Family members or substitute decision-makers who want to arrange dental care for a loved one and need a trusted provider.
THE REFERRAL PROCESS
Simple. Prompt. Professional.
We’ve designed the referral pathway to be low-burden. You don’t need dental knowledge to refer just a patient who needs care and a few basic details.
Submit a Referral
Use the form below or contact us directly. We need the patient’s name, location, a brief description of the concern or need, and consent confirmation (or a note that consent will be confirmed with the patient or their POA).
We Follow Up Promptly
We respond to all referrals within 1 business day. We’ll contact the patient, family member, or facility directly to arrange an appointment, and you don’t need to manage the scheduling.
Care Is Delivered On-Site
The SMDS team visits the patient at their home or care facility. An initial oral health assessment is completed and a care plan is discussed with the patient and, where applicable, their family or POA.
You Stay Informed (If You Wish)
With appropriate consent, we can provide a summary of findings to the referring professional. We’re committed to supporting the broader care team, not working in isolation.
Common Reasons Professionals Refer to Us
You don’t need a specific diagnosis to refer. If a patient’s oral health is a concern, that’s enough. Common presentations include:
Dental Pain or Discomfort
Patient is experiencing oral pain, swelling, or sensitivity but cannot access a clinic.
No Recent Dental Care
Patient has not seen a dentist or hygienist in years and requires an assessment.
Difficulty Eating or Chewing
Denture issues, tooth loss, or oral pain affecting nutrition and quality of life.
Cognitive or Mobility Barriers
Patient has dementia, mobility limitations, or anxiety that makes clinic visits impractical.
Pre-Surgery Oral Health Check
Patient requires dental clearance or oral health assessment before a medical procedure.
General Oral Health Concern
You’ve noticed something that warrants a professional dental assessment and want a trusted provider involved.
